All Birthday Club entries must be completed by a parent or legal guardian who is at least 18 years old. Children should not submit personal information directly.
A mailing address will only be requested from the selected child's parent or legal guardian after eligibility has been confirmed. Public birthday recognition will only occur with the parent's express permission.
